Community resources
Aging well at home is no easy feat. In fact, it likely takes the careful coordination of family members, friends, and healthcare professionals to successfully age at home. To better navigate this unique puzzle, we’ve put together a collection of resources for older adults in Iowa. Listed below are non-profit organizations that assist aging adults in locating resources, equipment and navigating the ever changing healthcare environment.
Happy at Home Consulting is excited to share our new partnership with the Iowa Department on Aging (IDA) to offer the CAPABLE program in our area. The program is being paid for by grant funding, so there is currently no cost to the participant.
CAPABLE is an evidence-based program to help Older Iowans decrease difficulties in daily living, reduce symptoms of depression, and support independent living. The service involves home visits from a Registered Nurse (RN), Occupational Therapist (OT), and one day’s work from a skilled handy worker. These visits aim to make aging in place easier and prevent falls by providing minor home modifications such as grab bars, tub benches, or improved lighting.
Eligible individuals include adults age 60 and better living in Dallas, Polk, or close surrounding counties. There is not an income restriction for this program and participants may own or rent their home. Interested individuals are encouraged to reach out to us to get started.
Referral partners may complete the screening application and send via fax or email.
