In addition to Medicare covered services, Happy at Home Consulting administers evidence-based programs throughout the state including the Home Hazard Removal Program (HARP). Shoe organizer added to remove floor clutter and improve access for independent dressing.
Bathing
Shower chair and tub clamp grab bar added for improved safety with showering.
Transfers
Transfer pole added for improved ease with getting in/out of the shower or on/off of shower.

Supplies reorganized to improve access to frequently used items.
Outdoor Access
Transition ramp added to accommodate height discrepancy.
Indoor Mobility
Transition ramp provided to improve wheelchair accessibility within home.
Home Hazard removal program (HARP)
The Home Hazard Removal Program (HARP) is a fully manualized, evidence-based fall prevention intervention designed to reduce falls for community-dwelling older adults. HARP includes:
- A comprehensive assessment of abilities, awareness of fall risk behaviors (Falls Behavioral Scale) and hazards in the home (Westmead Home Safety Assessment)
- A tailored home modification prescription developed collaboratively
- Assistance in removing the home hazards
- Education on fall risks and strategies to address fall risks

HARP as a formalized program presently available in Milestones AAA service area Appanoose, Clinton, Davis, Des Moines, Henry, Jefferson, Keokuk, Lee, Louisa, Lucas, Mahaska, Monroe, Muscatine, Scott, Van Buren, Wapello and Wayne Counties in Iowa.
CAPABLE is an evidence-based program to help Older Iowans decrease difficulties in daily living, reduce symptoms of depression, and support independent living. The service involves home visits from a Registered Nurse (RN), Occupational Therapist (OT), and one day’s work from a skilled handy worker. These visits aim to make aging in place easier and prevent falls by providing minor home modifications such as grab bars, tub benches, or improved lighting.
The CAPABLE program expanded to include Polk & Dallas counties in April 2022. We’ve had the pleasure of working with older adults in Central Iowa to reduce fall risk and maintain independence while living at home.
