Referral
Referrals and relevant history can be faxed to (515) 631-5210 or sent via email. Physician orders are not required for OT services.
Collaborate
If requested, work with our team to review the client's condition, goals, and any existing assessments to develop a plan of action.

The Home Hazard Removal Program (HARP) is a fully manualized, evidence-based fall prevention intervention designed to reduce falls for community-dwelling older adults. HARP includes:
- A comprehensive assessment of abilities, awareness of fall risk behaviors (Falls Behavioral Scale) and hazards in the home (Westmead Home Safety Assessment)
- A tailored home modification prescription developed collaboratively
- Assistance in removing the home hazards
- Education on fall risks and strategies to address fall risks
Ready to Go Home? is a specialized discharge planning service offered by Happy at Home Consulting, designed to help individuals safely transition from a hospital stay back to their home environment. Our licensed occupational therapists conduct comprehensive home assessments to evaluate safety, accessibility, and potential hazards. Based on these findings, we provide personalized recommendations, including home modifications and adaptive equipment, to ensure a smooth recovery and reduce the risk of falls or readmissions. Our goal is to support patients, caregivers, and healthcare teams in creating a safe and comfortable environment for recovery. This service is available without a doctor’s order and is priced at $250 for a one-time assessment.
At Happy at Home Consulting, we understand the importance of maintaining independence and physical function as you age. Our Home Exercise Support Program, developed in partnership with Cornerstone Caregiving, provides a personalized approach to help you or your loved one stay active and reduce the risk of falls. With guidance from our skilled Occupational Therapists and support from trained caregivers, we create individualized exercise plans tailored to your unique goals. Whether you want to improve strength, mobility, or overall wellness, our team is here to support you every step of the way. Together, we’ll help you stay safe, active, and healthy in the comfort of your home.
Happy at Home Consulting has spearheaded the creation of Home Modifications Iowa.
Home Modifications Iowa is a nonprofit organization supporting older adults, individuals with disabilities, and caregivers living in the community. The integrative approach connects individuals in need with existing community resources to find solutions to present needs while simultaneously planning for the future. Home Modifications Iowa provides adaptive equipment, assistive technology, and home modifications in an effort to help individuals remain in their home.
CAPABLE is an evidence-based program to help Older Iowans decrease difficulties in daily living, reduce symptoms of depression, and support independent living. The service involves home visits from a Registered Nurse (RN), Occupational Therapist (OT), and one day’s work from a skilled handy worker. These visits aim to make aging in place easier and prevent falls by providing minor home modifications such as grab bars, tub benches, or improved lighting.
The CAPABLE program expanded to include Polk & Dallas counties in April 2022. We’ve had the pleasure of working with older adults in Central Iowa to reduce fall risk and maintain independence while living at home.

Additionally, we offer organizational support to help implement these programs smoothly and successfully, enhancing their overall impact.
